The Azdaran document appears in your inventory when you find all 10 Azdaran fragments in the Ancient Keldagrim ruins north of Keldagrim library, which can only be mined after completing Birthright of the Dwarves. Receiving the document will complete the Azdaran Birthright achievement, which is a master quest cape requirement. The document can be taken to the librarian in Keldagrim Library to discuss it. It may be placed on a bookshelf in the player's house.
The Mining level needed (can be boosted), and experience rewarded is as follows:

The dark god Zamorak has corrupted the magic that Guthix placed in our blood, such that all dwarves will slowly transform into twisted abominations.
