The passive has a 1/30 chance per hit to activate. When it activates, this will instantly kill the monster, or it will deal up to 5,000 damage to bosses. The effect doesn't work on untaggable monsters.
The book can only activate when the target monster has a high proportion of life points - it won't activate if the monster has been significantly damaged.
The book is most effective when killing non-boss monsters with high lifepoints (>30K), such as (from highest to lowest lifepoints): Rune Dragons, Hydrix dragons, Acheron mammoths, Onyx dragons, Soulgazers, Dragonstone dragons, Bulbous crawlers, Moss golems, Nightmares, Ripper Demons, Adamant Dragons, Edimmu, and Vinecrawlers.
Essentially, it is always more effective to use a God Book, Berserk blood essence, or damage-/accuracy-boosting scrimshaw when fighting bosses, and more cost-effective to use Attacker's insignia when fighting low level monsters.
