The Defender of the Mind is an outfit override awarded by completion of the Gielinorian Giving time-limited event. It was earned part-by-part by turning in Charity tokens to Kat, Zoed, or Lau'Ra.
The outfit and the cape were also available for 195 RuneCoins each during the Gielinorian Giving II event and the Mental Health Awareness Week.
