Double eyepatches are the result of having Patchy sew a left eyepatch and an eyepatch together. Left eyepatches are lootable items from Pirate implings, which can be caught after the Rocking Out quest. Right eyepatches can be obtained from killing low-level pirates, or bought at Trader Stan's Trading Post or at the Fancy Dress Shop.
On the equipment screen it looks like 2 different eyepatches wrapped around their head; the bands make an "X" pattern on the back of the head.
These can be bought from another player, but not on the Grand Exchange. The trade value is the price of an eye patch and a left eyepatch added together.
