A Pirate's hat can be obtained from doing level 3 Treasure Trails, bought from another player, or bought on the Grand Exchange. The Pirate hat is part of Pirate equipment, and is commonly worn with Pirate boots and Pirate's hook, and doesn't give any stat bonuses.
It can be attached to Double eyepatches by Patchy on Mos Le'Harmless for 500 coins, giving the Pirate hat and eyepatches. It can also be combined with an eyepatch or left eyepatch the same way.
