The Warden of the Mind is an outfit override awarded by completion of the Gielinorian Giving II time-limited event. It was earned part-by-part by turning in Charity tokens to Kat, Zoed, or Lau'Ra.
The outfit and the cape were also obtainable for 195 RuneCoins each during the Mental Health Awareness Week event.
